JOB DESCRIPTION
Title: In-Home Support Specialist
Reports to: O.A.R.S. Service Coordinator
JOB SUMMARY
This position is responsible for carrying out services as detailed in the customer’s plan of care and/or individual service plan. These forms of assistance may include help with chores, personal care, transportation, providing a break for the primary caregiver depending on the needs of the customer as outlined in the plan of care and/or individual service plan. All services are provided to customers who experience a level of disability that causes them to be at risk of having to be institutionalized.

1. Able to implement services as planned in partnership with families and other team members.
2. Have strong communication skills with diverse populations including families, team members and the public.
3. Contributes to the professional reputation of the agency.
OTHER QUALIFICATIONS
Must have a high school diploma or GED and be at least 18 years of age. All applicants must be able to pass a criminal background check under state regulations and be able to obtain First Aid/CPR training within one month of being hired.
Performance Standards for In-Home Support Specialist
1. Implements plan in partnership with families and other team members. (KSA)
Function: Implements plan in partnership with families and other team members.
Standards:
a) Is aware of and adheres to all elements of the plan of care and/or individual service plan.
b) Accesses resources necessary to implement the services detailed in the plan of care and/or individual service plan.
c) Implements plan in a manner sensitive to the customer, family and community.
d) Makes adaptations according to specific medical needs.
e) Maintains mutually respectful boundaries with families.
f) Interacts with other service providers in a professional manner.
2. Has effective written/oral communication skills with diverse populations including; families, team members and the public (KSA)
Function: Communicates with families in a positive manner.
Standards:
a) Seeks regular feedback from families regarding the effectiveness of services.
b) Adapts to communication styles of individuals from different cultural backgrounds.
Function: Written communication (service notes, incident reports) is clear, concise and timely.
Standards:
a) Service notes are completed daily, are legible, meet Medicaid regulation standards and are submitted on time.
b) Incident reports are completed as needed and submitted as soon as possible to supervisor.
3. Is dependable and follows through on commitments (KSA)
Function: Practices effective communication with supervisor.
Standards:
a) Seeks clarification and follows through on expectations regarding which actions/changes must be approved by supervisor.
b) Listens and asks for clarification when necessary.
c) Avoids defensiveness and seeks mutual understanding with supervisor.
d) Give feedback to supervisor in a constructive way.
4. Contributes to the professional reputation of the agency. (KSA)
Function: Adheres to agency policies and procedures.
Standards:
a) Locates and refers to agency policy manual
b) Asks for clarification regarding policies/procedures from supervisor.
c) Complies with all agency policies and procedures.
