| Our Mission: We provide individualized services for adults with physical disabilities and seniors in partnership with families to assist customers in meeting their goals for living with dignity in their chosen homes and communities. Services may include respite care, chore services, care coordination, transportation and personal care services. |

| All of our services are created and designed by the customer, family and our Care Coordinators with the goal of meeting specific individual needs. What services are available? • Care Coordination Services to help identify and obtain available services in the community of choice. • Respite Services to relieve caregivers so they can relax and attend to their own personal needs. • Chore Services to help with errands, house cleaning, meals, and other chores. • Personal Care Services to assist with personal care needs like bathing, dressing, exercise, and medication assistance. Why are services important? • They help people take control of their own care. • They allow people to live in their own homes and community of choice. • They decrease stress within the home. Payment for services Community Connections accepts payment from: • Medicaid • State Grant Funds • Private payment |
